Q: What do I need to bring to my appointment?
A: Current insurance card, a photo
ID, current medication list, known allergies, and Nebraska Pain
Consultants Evaluation Form (click form and print).
Q: Do I need any information from
my family physician?
A: Yes, we need your medical records, referring physician notes,
MRIs, CT Scans, and Nerve Conduction Studies. Call your doctor
to have them sent to us or pick them up and bring them with you. Your
doctor’s office may FAX this information to Nebraska Pain Consultants
at 402-323-8599.
Q: What if I don’t bring my medical
records?
A: Without the information from previous providers
it is very likely your appointment will be rescheduled.
Q: Should I arrive for my appointment
early?
A: If you have not completed the Nebraska Pain Consultants
Evaluation Form then arrive at least 15 minutes early for assistance
to complete this information.
Q: What if I am late?
A: Nebraska Pain Consultants strives to see you in a timely
manner. If you are more than 15 minutes late then your appointment
will be rescheduled to a later date.
Q: What if I need to change my appointment?
A: We require a 24 hour notice for cancellations. After 3 missed
appointments without cancellation notice you will be dismissed
from Nebraska Pain Consultants patient list and will need to ask
your physician to refer you someplace else.
Q: Will I receive an injection on my initial appointment?
A: No. Your initial appointment is to determine your medical
history, perform a physical exam, and assess your pain. If after
examination an injection is recommended for your condition you
will be scheduled to receive it on a future date. You will be given
specific instructions concerning the preparation for this pain
management service.
Q: Will my appointment need pre-authorization?
A: Some insurance companies require pre-authorization. If
you are not sure then ask your human resource department.
Q: What if I’m covered by General Assistance?
A: All General Assistance patients must be pre-authorized.
Q: What if my visit is a work related injury?
A: Approval from your Workers Compensation case manager
will be necessary for your initial and all future appointments
with Nebraska Pain Consultants. Approval must be obtained prior
to your visit. Ask your case manager if a Workers Compensation
form needs to be completed for your file.
Q: Does my insurance cover my appointment?
A: Nebraska Pain Consultants services are generally covered by
a number of health plans including: Blue Cross Blue Shield, United
HealthCare, Midlands Choice, and Mutually Preferred. We are currently
working with other health care providers. Please contact our offices
at 402-323-8484 for specific questions about insurance.
Q: Do you bill my insurance company?
A: Yes. If you have an office visit co-pay or deductible
for these services you should pay at the time of service or we
may bill you.
Q: What if I don’t have insurance?
A: Nebraska Pain Consultants requires a $100 payment at the time
of service. If the charge is more than the payment you will be
billed for the remainder.
Q: What if my visit is the result of an automobile accident or
other circumstances where my regular health insurance is not responsible?
A: Nebraska Pain Consultants requires a $100 payment at the time
of service for evaluation and $300 for treatment. If the charge
for your services is more then you will be billed for the remainder.
In the event the charge is less, you will receive a refund. |
